PHM 325 Practice Cases
1.Review the patient case, clinical goals of care for type 2 diabetes, and algorithm for use of injectable medications to treat type 2 diabetes presented in the images below. Evaluate if the patient is currently achieving their fasting (preprandial) blood glucose target. Based on your assessment, would you recommend to continue the patient's current dose of basal insulin (Basaglar), increase the dose, or decrease the dose? Please write a plan in regards to the patient's basal insulin dose. Make sure to include the drug name, dose, route, directions, etc.
2.EH's primary care physician suggests that we continue to follow the next step of the injectable medication titration algorithm by adding on a GLP-1 RA (receptor agonist), such as the drug Victoza, to assist with achieving the patient's clinical goals. Utilize your drug information resource to review the contraindications, warnings, and precautions for Victoza. Is EH a candidate for this medication? Why or why not?
3.Utilize a drug information resource to review the dosing information for the patient's oral diabetes medications (metformin and canagliflozin). Compare this information to EH's available laboratory parameters. Describe the MTP that you identify and present a plan to resolve it. Include monitoring parameters for the medication in your plan.
4.Evaluate EH's patient case for the following questions related to adherence. Does an MTP currently exist? If so, describe the MTP and present a plan to resolve it.
- 15) Are there any medications the patient is unable to access, afford, or take as directed for any reason?
- 16) Are there any medications the patient is missing doses of or taking incorrectly?
- 17) Is it possible to reduce the patient’s pill burden or streamline adherence?
5.Review the patient case and clinical treatment algorithm provided below. What medication therapy problem do you identify for this patient? Why?
6.Four antimicrobial options are recommended as first-line agents for the treatment of uncomplicated cystitis. Two are noted to be less effective than other options and can be ruled out. Utilize your drug information resource to determine which of the remaining two options would be the most appropriate recommendation for this patient. What would be your plan for starting medication therapy?
